ETHICS HEALTH SERVICES AND TRADE (INTERLIVA) DISCLOSURE TEXT ON THE PROCESSING OF PERSONAL DATA EMPLOYEE

As Etik Sağlık Hizmetleri Ve Ticaret Anonim Şirketi (hereinafter referred to as “INTERLIVA”), we hereby submit for the information of the public and relevant persons the clarification text we have prepared in accordance with Article 10 of the Law No. 6698 on the Protection of Personal Data (“Law”), which contains information about the personal data processing activity specific to the recruitment activity;

ARTICLE 1: DATA CONTROLLER

Your personal data may be processed by INTERLIVA as the data controller within the scope described below. What should be understood by the concept of data controller is the natural or legal person who determines the purposes and means of processing personal data and is responsible for the establishment and management of the data recording system.

d- Identity / Fulfillment of obligations arising from the employment contract and legislation for employees, carrying out training activities, planning human resources processes, carrying out storage and archive activities, providing information to authorized persons, institutions and organizations, carrying out management activities, carrying out fringe benefits and benefits processes for employees

ARTICLE 4: PERSONAL DATA COLLECTION METHODS AND LEGAL GROUNDS

Personal data of employees are processed through the physical delivery of documents by the relevant person. The legal reason for the collection and processing of personal data is as follows for paragraphs 2/c, 2/d, 2/e, 2/f, 2/h and 2/h of this clarification text:

a- Art. 5/2-c of the Law No. 6698 on the Protection of Personal Data / It is necessary to process the personal data of the parties to the contract.

The legal reason for paragraphs 2/a, 2/b and 2/g is to obtain explicit consent.

ARTICLE 5: RETENTION PERIOD OF PERSONAL DATA

The retention periods of the personal data written in this clarification text according to the data categories are listed below and the personal data is destroyed at the end of the period.

a- Criminal conviction and security measures / Since it is within the scope of the personnel file, 10 years from the termination of the employment relationship

b- Philosophical beliefs, religion, sect and other beliefs / 10 years from the termination of the employment relationship

c- Contact / 10 years from the termination of the employment relationship

d- Identity / 10 years from the end of the employment relationship

e- Professional experience / 10 years from the end of the employment relationship

f- Accounting / 10 years from the end of the employment relationship

g- Health information / 10 years from the end of the employment relationship, 15 years for health reports received for occupational safety

h- Personnel / 10 years from the termination of the employment relationship

i- Fingerprinting / 10 years from the termination of the employment relationship

j- Your Visual Data / 10 years from the termination of the employment relationship

k- Auditory Recordings / 10 years from the termination of the employment relationship

l- IP address information, website login and exit information, password and password information, your information processed for the management of commercial, technical and administrative risks / 10 years from the termination of the business relationship

m- Your Location and Vehicle Data / 10 years from the termination of the business relationship

ARTICLE 6: RIGHTS OF THE PERSON CONCERNED

The natural person whose personal data is processed is defined as the data subject and has the following rights by applying to the workplace:

a- Learning whether personal data is being processed

b- Request information if personal data has been processed

c- To learn the purpose of processing personal data and whether they are used in accordance with their purpose

d- To know the third parties to whom personal data are transferred domestically or abroad

e- To request correction of personal data in case of incomplete or incorrect processing

f- Request deletion or destruction of personal data

g- Request notification of the transactions made pursuant to subparagraphs (e) and (f) to third parties to whom personal data are transferred

h- Object to the occurrence of a result to the detriment of the person himself/herself by analyzing the processed data exclusively through automated systems

i- In case of damage due to unlawful processing of personal data, to demand the compensation of the damage
